จากเหตุการณ์ที่ผ่านมาทั้งสองภาค เหล่าแวมไพร์ไม่เคยเอาชนะนักล่าอย่าง เบลด (เวสลีย์ สไนป์) เจ้าของฉายา The Daywalker ได้เลยสักครั้ง กับดักสุดท้ายที่เบลดต้องเผชิญก็คือ เดร็ค (โดมินิค เพอร์เซลล์) ราชันย์แห่งแวมไพร์ ที่ถูกพัฒนาจนถึงขั้นสุดยอดยากต้านทาน, ผู้กุมโครงสร้างหลักของโลกมนุษย์ แผนของเดร็คคือ การปล่อยข่าวสร้างให้เบลดกลายเป็นผู้ต้องสงสัย ในคดีฆ่าต่อเนื่องอันสยดสยอง!! และเป็นโชคร้ายของเบลด เมื่อทุกคนหลงเชื่อ!! ทั้งเหล่ามนุษย์และแวมไพร์ ต่างระดมกำลังเพื่อตามจับเขา จึงถึงคราวที่เบลดต้องยอมรับความช่วยเหลือจาก The Nightstalkers ทีมมนุษย์ล่าแวมไพร์ที่เขาเคยดูถูกอย่าง แฮนนิบาล คิง (ไรอัน เรย์โนลด์ส) และ อะบิเกล วิสเลอร์ (เจสสิก้า บีล) ลูกสาวของ ดร.วิสเลอร์ ผู้มีพระคุณของเขานั่นเอง…
